Where are you located?

Our address is 370 Lexington Avenue Suite 2300. We are one block south of Grand Central Station, at the corner of 41st Street. The closest subway lines are the 4, 5, and 6 – exit at Grand Central Station/42nd Street. The closest bus is the M42 (Lexington Avenue line).

What are your office hours?

Monday – Thursday, 7:00am – 8:00pm

Friday, 7:00am – 5:00pm

We accommodate weekend and home visits at off hours by appointment only.

Do you accept insurance?

PHLEX is an out-of-network provider. Our staff is happy to verify your benefits and advise you of your cost per visit and the claim submission process. We do not participate with Medicare, Medicaid, Workmen’s Compensation or Personal Injury/No-Fault claims.

What should I expect on my first visit to PHLEX?

Your first visit will be an evaluation session. Within this session, the following will occur:

  • An in-depth conversation between you and your clinician, reviewing your history, current symptoms, and your personal goals. At this time the clinician will also review any paperwork from your doctor with you.

  • An extensive physical examination, which may include any of the following as deemed appropriate by your clinician: range of motion, flexibility, strength, functional, postural, and mobility patterns

  • Your first treatment as well as a home exercise program.

  • An individualized and focused plan of care based on your needs as established during the evaluation.

What should I bring to my first visit?

Please be prepared to bring the following to your first appointment:

  • Doctor’s prescription (if you have one)

  • Insurance card (if you are planning to use your out-of-network physical therapy benefits)

  • Credit card

  • Any relevant imaging or diagnostic testing (MRI reports, x-ray reports, EMG reports, etc)

  • Any forms you have completed at home if you did not utilize the online patient portal

  • Comfortable gym/fitness clothing. For physical therapy patients, clothing should allow your therapist access to the injured body part.

  • If you prefer to complete your paperwork upon arrival to our office, please plan to arrive 10-15 minutes prior to your scheduled appointment. We value your time and want you to take full advantage of every minute of your appointment!

Will the practitioner consult with my doctor? How about my personal trainer?

Certainly! Our practitioners are happy to facilitate any coordination of your care with your permission. Your doctor will be sent a report after your initial evaluation, and will also be sent progress reports throughout your course of treatment.

How long are the sessions?

The majority of sessions are 50 minutes. However, for some services we do offer 30 minute sessions.

I haven’t seen a doctor yet. Can I still come to PHLEX for Physical Therapy?

In most cases, yes! In 2006, Congress passed legislation that allows patients in New York State who need Physical Therapy to be evaluated and treated by an experienced Physical Therapist for up to 10 visits or 30 days without a prescription from a doctor. Upon verification of your benefits, our staff will let you know if your insurance will cover Direct Access visits. If you see a PHLEX Physical Therapist under Direct Access, and it is determined that further examination by a physician and diagnostic testing are necessary, we will communicate with a physician of your choice, or recommend the specialist we think is the right match for you.
